GOALS
Three levels, deliberately
They are meant to connect.

Each goal carries a current value against a target, so progress is a number rather than an opinion. Goals are flagged as active, at risk, achieved or abandoned.

Tip
At risk is the useful state, and the one people avoid using. A goal that is quietly not going to happen is worth flagging while there is still a quarter left to do something about it — that is the entire purpose of tracking them.
Watch out
Abandoned is not a failure state to be avoided. A goal the business deliberately stopped pursuing should be marked abandoned rather than left active and missed, or the next review cannot tell the difference between changing your mind and not delivering.
FEEDBACK
Between the reviews
The part that makes the review possible.

Feedback can be recorded as it happens rather than reconstructed at review time. A review written from six months of notes is a different document from one written from memory in an afternoon, and the difference is obvious to the person receiving it.

The cycle
And why calibration exists.

Reviews run as a cycle across the people in scope. Where a workspace has appointed calibrators, they look across reviews before anything is final — because different managers rate differently, and comparing people fairly means correcting for that rather than pretending it is not happening.
